Fiddler's neck

Fiddler’s neck, sometimes called violin hickey, is a skin condition that comes from playing the violin or viola. It shows up as redness, thickened skin, and swelling on the left side of the neck, just below the jawline, where the instrument presses. The area can also develop acne-like bumps, cysts, and infections.

The main causes are constant rubbing and pressure from the instrument. Poor hygiene can lead to infections, and hot weather, tiredness, emotional playing, or performing in small, high-stress groups can make it worse. Allergies to materials like rosewood, ebony, varnish, rosin, or metal clips on the chin rest may also play a role. The condition is well known among professional orchestra musicians but is often overlooked by dermatologists. In fact, a red mark on the left side of the neck under the jaw is considered a telltale sign that someone is a violinist or violist, even without seeing the instrument.

Not every player develops fiddler’s neck. Skin sensitivity, playing habits, and the materials used in the instrument all affect whether it appears. Under the microscope, the skin often shows thickening of the outer layer, clogged hair follicles, and collections of immune cells reacting to foreign material—usually tiny wood particles from the chin rest.
